Determining when it is 'time' is never an easy task. However, we like to emphasize that our ultimate goal should not be to squeeze out as much physical time as possible with our pets, as much as we might want to, as it is to ensure that our pets are experiencing quality life until their very last breath. What were they like as a puppy? As a young adult? What has been important or unhelpful to your pet throughout their lives? Below is a scale that may help you more clearly determine where your pet is at. The doctor will also discuss this in more detail during your introductory phone call and ultimately your appointment.

INTAKE FORM

To be considered for an appointment, please fill out our intake form. Although we know it is a challenging time for your family, more information is always better, so be as detailed as you are able. Feel free to describe what you and your pet have been going through to get you to this point, and what treatments you have tried for this condition. You can also includ additional details like who will be present, and whether you would like to have the procedure done in your home or in another location such as your backyard, or a nearby park.
